Since no one better equipped to translate this for you, I'll take a stab at it.
Some of the clauses do not seem to connect properly and I may not have all the words or sometimes the correct word, but you'll get the gist, I think:
"The state board of insurance at Kaunas
The Regional inspectorate of [svyriu county?]
Evidence of fire insurance
The location of the building is insured for the period from 12 midnight of December 31, 1941, until 12 noon on January 1, 1943.The the sum of insurance [will be?] determined according to the governance of the insurance.The base of the insurance comprises the common conditions obliged by the fire insurance.The certificate of insurance on deposit is backed by a pledge to pay."
Now I've taken a good deal of liberty with some of the text because it is written in a formalistic, perhaps even legalistic, manner that I'm not used to and so may not have translated well.However, it is clear enough that this is a fire insurance policy, perhaps the cover sheet of it, and that the buildings insured were probably in the Kaunas district -- the "svyriu apskr." may point to the specific location.
So there's my shot at a translation.I hope this helps a little.
