Have received the SS-5 application for a SSN for FRANK DUNHAM LEEDS dated July 2, 1937. He lists his address as 1412 Highland Ave., Lakewood, Ohio. He lists age at last birthday as 59, DOB as July 9, 1876*, Place of Birth as Georgetown, Ohio; parents Thomas Jefferson Leeds and Mary Ann Smiley. The SSN assigned was 294-07-1547.
*the year of birth appears to be originally written as 1876, crossed out by another hand to say 1877. Also the business name of the employer was entered by someone else as the City of Lakewood.
