I, Jacob Laire, desiring to volunteer as a soldier in the Army of the United States, for the term on one year, Do declare That I am 42 years of age; that I have never been discharged from the United States service on account of disability or by sentence of a court-martial, or by order before the expiration of a term of enlistment; and I know of no impediment to my serving honestly and faithfully as a soldier for one year.

in the state of Ohio, aged 42 years, and by occupation a farmer Do Hereby Acknowledge to have
volunteered this 22nd day of February, 1865
to serve as a soldier in the Army of the United States of America, for the period of
one year, unless sooner discharged by proper authority: Do also agree to accept such bounty, pay, rations, and clothing, as are, or may be, established by law for volunteers.
And I’, Jacob Lair do solemnly swear, that I will bear true faith and allegiance to the United States of America, and that I will serve them honestly and faithfully against all their enemies or opposers whomsoever; and that I will observe and obey the orders of the President of the United States, and the orders of the officers appointed over me, according to the Rules and Articles of War.

I CERTIFY, ON HONOR, that I have minutely inspected the Volunteer, J. Laire previously to his enlistment, and that he was entirely sober when enlisted; that to the best of my judgement and belief, he is of lawful age; and that in accepting him as duly qualified to perform the duties of an able-bodied soldier, I have strictly observed the Regulations which govern the recruiting service.
This soldier has blue yeyes, brown hair, dark complexion, is five feet 7 1/2 inches high.

Declaration for Original Invalid Pension
State of Ohio County of Scioto }S.S.
On this 10 day of July, A.D, 1890, personally appeared before me, A.M. ?__?., a notary public in and for County and State aforesaid, Jacob Laire, aged 70 years, a resident of (blank) County of Scioto State of Ohio, who being duly sworn according to law, declares that he is the identical Jacob Laire who entered service during the War of the Rebellion under the name Jacob Laire on or about the 13 day of Feb, 1865, as Pvt., in Co. A of the 195th regiment of Ohio commanded by Thomas Lewis, and was honorably discharged at Columbus, Ohio, on or about the 22 day of December, 1865, by reason of Expiration of ?Mar.?; that his personal description is as follows: Age 45 years height, 5 feet 10 inches; complexion, light; hair light; eyes, blue. That he is now suffering Rheumatism and Lung disease, and the said disability is of a permanent character, and is not the result of vicious habits, and that it incapacitates him from the preformance of manual labor in such a degree as to render him unable to earn a support, and that this declaration is made for the purpose of being placed upon the pension roll, under the provision of the Act of June 27, 1890. That he has not been employed in the military or naval service otherwise than as stated above.
That his Postoffice address is Lilly, County of Scioto State of Ohio
