Needing help with Early Union Parish History Please

I have an old bible record and in it reads the following besides the names and dates.

Union Parish LA the AD 1852
the Baptist Church here at Provider hereby certify
that our beloved sister Anice/Unice Beardin
is a member of our Church in full community
with us and is at her request hereby,
dismised from us, but subject to our disciplin
until joined to some other church of our order,
signed by order of the Church C H Baily (CC)
J E Martin (Mis?).

Does anyone know if their was a Baptist Church or town by the name of Provider??
Or anything about C H Baily or J E Martin

Eunice ? Kimball Bearden was the 2nd wife of Arthur Bearden b abt 1810 in GA lived 1850 and 1860 in Union Parish La
Some of his kids are buried in Sardis Cemetery
Her first husband was John Stanaland "Jackey" KImball
son of Joseph Kimball b 1774 and Rebecca Murray b 1774

John Kimball died 8 Nov 1849
Eunice was not on the 1850 census with Arthur

But her and Arthur had a son named Lenard T/F Bearden b 19 May 1852 and d 15 Nov 1852

I have not found the burial place of none of them.

I think Eunice moved from Union Parish based on this but am not sure

the bible says she died 21 May 1855 possibly at the age of 35.

We dont know her maiden name.

Any help would be greatly appreciated
Debra TX
