You can accept his birth info on his death cert...there's no need to be precise, other than needing to cross all "T" and dot all "I"Do you have the DC and/or obit? [obit would be in the Paonia Public Library].If YOU obtain enough of the SAME circumstantial info [not from others] then it is usually accepted.From what little I've searched, his parents appear to be John & Grace HAZLETT of Bourbon Co KS.I also noted on several census the name is Harry W B, or H W B.I saw your posting of "Who's Who In American Medicine"...a great resource!
I had read your msgs elsewhere stating you were having trouble identifying John's parents.
There are 3 John HAZLETTs b 1831-33 PA;2 in Allegheny Co; the other in Columbia Co.Maybe John's 8-10-1914 DC might name his parents.He d/bur Leavenworth Soldiers Home.
You probably know most of this...Grace lived in Cedar Co MO at time of John's death; she d 4-22-1928 Santa Barbara CA; she also filed for John's CW Pension [usually has great info]; resided 1920 Monterey CA
