Try: Historical Register of National Homes for Disabled Volunteer Soldiers, 1866-1938; (National Archives Microfilm Publication M1749), Records of the Department of Veterans Affairs, Record Group 15; National Archives, Washington, D.C. Register S. Stamp on upper right of page is 21540 (page number, I think).

If you go to the Nat'l Archives and access Ancestry.com on one of their computers (free), you can easily find this record in the military databases. Lots of info.
