Terri,

The paper you have of your Dads is called a DD214. You should piece it together. It will have his service number on it. On your search engine or go to Google.com and type in Standard Form 180. Print out both sides and mail it to St.Louis the address is on the back side of the SF180. If you know his social security number that will help. It is fairly easy to fill out. Takes a few months to get things back as there is a backlog. You can get medals and his records with this form.
