Death certificates in Japan

I am attempting to obtain a copy of a death certificate for a Japanese national who died in Japan. Can anyone provide some guidance on how to go about this? The Japanese Embassy in the USA only told me I had to contact the local government in Japan.

The last home address I have for the deceased is Hyogo Prefecture, Muko Gun, Motoyama Mura, Tanaka 79

Does this make any sense? I can't locate this address on a map of Japan. Hyogo Prefecture is clear enough, but what is the town?
