You can go to the NARA site and find the distict office that covers Chicago...and e-mail them with a request for the documents. Since you have the number of the actual paperwork, should not be difficult to find.
You can also go to the www.familysearch.org site and look for Illinois records for Naturalization. The declarations of intent are one microfilm for both the Northern and Southern district of Illinois...not sure which district your papers are in. But you can order the microfilms and get the info that way as well as contacting the NARA...
