I think this is an example of Lays for Death or Death Poems. Probably anonymous, like this one, commonly put on tombstones or on samplers:



Forget thee not while life does last
and when this life is o'er
My spirit shall revisit scenes
so dearly prized before
shall watch beside thee many an hour
though thou perceive it not
and on thy meditative brow
read-
I am not forgot

But, and this is a long shot, I swear one of the lines in yours reminds me of something I read a long time ago in Beowulf, which is an epic poem. Maybe you could search online at Project Gutenberg, here:
