Border Crossing Form: Relative's Name, Address

I have a US border crossing form for SS Pisa, departing Hamburg, 23 April 1913. In the column for "name and address of nearest relative or friend in country whence alien came", each row is empty except for numbers in this format: #/#. I presume this refers to the location where the information is contained. What is this location?
I have checked the Canadian Passenger Manifest for this ship: SS Pisa, departing Rotterdam, 25 Apr 1913, arriving Quebec, 5 May, 1913. It does not have the name and address information.
