What you are referring to is called the Marriage Application. In many County Clerk's offices in ILlinois, these are kept in a separate volume. If you write the County Clerk and explain that you are seeking the application form for the marriage license, they should be able to tell you if they still have them. Some County Clerks will treat that as a separate purchase. Others include it with the license, perhaps depending on how they were kept.
