Illinois Adoption Registry

If he registered with the State of IL, "Illinois Adoption Registry" to attempt to contact family, and if someone from the family also registers an attempt to contact him, the State of IL will put you in contact with each other.

The necessary forms can be printed from the website.
There is no fee to register with the "IL Adoption Registry" if you also fill out the Medical Information Exchange form, at the time of registry.

The website states:
"Is there a fee for registering with the Adoption Registry?
Yes. There is a $40 filing fee. HOWEVER, THE FEE WILL BE WAIVED IF THE REGISTRANT COMPLETES the Medical Information Exchange Questionnaire at the time of the initial application and agrees to the release of the medical questionnaire to at least one other party to the adoption."
