Found some further info:
"Roster & Record of Iowa Soldiers in the War of Rebellion"

Hezekiah Fisk
Residence: Indianola
Enlisted - 10/19/1861 as Private
On 11/25/1861 he mustered into "G" Co., 15th Infantry
He died of wounds on 8/19/1864 at Atlanta, GA

He was listed as:
1) POW 4/6/1862, Shiloh, TN
2) Wounded 8/17/1864, Atlanta, GA (severe left shoulder and side wounds)

Promotions:
1) 1st Lieut 8/1/1862
2) Asst Surgeon 3/7/1863

Born: Indiana

You'll note that his headstone has the date that he enlisted into the military as his death date.
