In the Iowa State Census of 1905, the family is in Guthrie Center:
F. J. McMillen
Edith McMillen
Keith B. McMillen
Walter R. McMillen
Homer D. McMillen
Ruth B. McMillen

In the 16 June 1900 census, the family was also living at Guthrie Center, so it seems unlikely that Belle was born in Avoca. I'd guess she was born at Guthrie Center:

Francis J. McMillen, male, b. June 1855, head, married 17 yrs, b. IL, father b. OH, mother b. OH, farmer
Edith E. McMillen, female, wife, b. Jan 1864, b. Iowa, father b. OH, mother b. MO
Buell K. McMillen, son, b. Jan 1885, b. Iowa
Rodney W. McMillen, son, b. Feb 1886, b. Iowa
Homer C. McMillen, son, b. Aug 1891, b. Iowa
Bell McCormick, mother-in-law, Nov 1825, age 74, widow, b. MO, father b. OH, mother b. MD
