You are right, 99% of the time.
I've had Greek family (ppl who hadn't come to the US) who changed surnames, though it is unclear if anything was ever done legally because we are talking about a time (pre 1912) when things weren't as organized as they are now (and even now I shake my head .. but that's another story)
Some families had nicknames to accompany their family name, sometimes it stuck sometimes it didn't, sometimes 1 branch would keep the nickname while the other branch would keep the original surname..
