This looks like her death certificate although the date of death is 9 August 1976:

She is the second one on the page, click on her name and you'll have the info. You may also view the original. Note that the informant is her mother Brenda but the last name is spelled "FORSTON" yet the infant is listed as "FORTSON", a typo or...? Name of father is not given on the death certificate. Hope this helps.
