It sounds as though you may have edited the death fact's sentence structure by accident.

Go to the "Lists" menu, select "Fact Types" and find "death," edit it and change the sentence structure. You can choose the pronoun or name to begin the sentence and the connecting words, but Family Origins puts the date and place in for you, as the demo sentence shows you.
