Sarah Maddox Floyd 1824-1890

Can anyone tell me the source of Maddox in Sarah's name?

PLEASR HELP: Does anyone have ANY kind of evidence of Sarah's parents?
Census records indicate she is probably the daughter of Jabez Floyd 1791 and Nancy Douglass Floyd rather than Lewis and Nancy Floyd. (see below)
