Nine years ago (May 2000), I sent for and received from the National Archives, Jacob Eversole's pension file for service in the War of 1812.I have since passed this on to another researcher, but I saved extensive notes as follows:

State of Ohio, Sandusky County, ss:
On this ninth day of November A. D. (1850) personally appeared "Jacob Eversole aged fifty nine years, - a resident of said county and State aforesaid - who being duly sworn according to law, declares that he is the identical Jacob Eversole, who was a Corporal in the Company of Riflemen, commanded by Captn David Ewing in the Regiment commanded by Major Vance and Col.----- Wells in the war with Great Britian (sic), declared by the United States, on the 18th day of June 1812 - that he volunteered (he thinks) near Somerset, or in Muskingum Co, Ohio, about the----- day of April A. D. 1813, for the term of------and continued in actual service in said war, until the fifth day of August, A. D. 1813,"- honorably disch at Lower Seneca OH on same day..... as will appear from "my discharge herewith enclosed."He makes this declaration for the purpose of obtaining the bounty land to which he may be entitled.....

Court clerk Rawson certified that John Bell is a JP in and from said county, etc.....
A note at the bottom shows "Please address me, as above, Saml Crowell."
ITEM DATED 5 MAY 1851 .....No 18.255 ....form letter from Treasury Department, Third Auditor's Office to Commission of Pensions, Department of the It appears from documents on file in this office that Jacob Eversole a Corporal of Captain David Ewing's Company of Ohio Militia, entered the service on the 8th April 1813 (indef), and served till the 5th August 1813, the date of discharge.
ITEM DATED 3 APRIL 1855- this is a handwritten affidavit:
State of Ohio, Sandusky County-on this 3d day of April (1855), Personally appeared before me John Bell a Notary Public.... Jacob Eversole aged 63 years, res: Sandusky Co OH, swears he is the Jacob Eversole who was a Corporal in the Company commanded by Capt David Ewing in the Regt commanded by C ------- Wells in the war with Great Britain declared by the United States on the 18th day of June 1812 for the time of six months and continued in actual service in said war for the time of Fourteen days, that he has heretofore made application for bounty land under the act of September 28, 1850, and received a land warrant...for 80 acres which he has disposed of....He makes this declaration for the purpose of obtaining this the additional bounty land to which he may be entitled to under the act of 3 March 1850..... DECLARATION FOR WAR 1812 ARMY PENSION
State of "Indiana"County of "Elkhart" ss:
On this "14" day of "March" A. D. one thousand eight hundred and "seventy one" personally appeared before me, "La Porte Heefner" Clerk of the Circuit Court within and for the County and State aforesaid, "Jacob Eversole" aged "79" years, a resident of "Harrison Tp" in the County of "Elkhart" in the State of "Indiana" who being duly sworn according to law, declares he is the identical "Jacob Eversole," who enlisted in the service of the United States at "New Lancaster," County of "Fairfield" State of "Ohio" on the "first week" (printed day is crossed off form) of "April" in the year 18"13," as a "Corporal" in Company commanded by Captain "David EwingMajor Vance" in the "--------" Regiment of "Colonel Wells, of General Harrisons Army" in the war of 18"12""That his Term of enlistment was for six months" and was honorably discharged on the "Fifth" day of "August" in the year 18"13;"..... "at" a place called "Lower Senaca near Lower Sandusky" in the State of "Ohio"..."that He received a written discharge from said service which was forwarded to the General Land Office of the United States when he applied for & received 160 acres of Land under act of year 1850That the applicant was married to his present wife, now living, whose maiden name was Barbara Bixler, at Fairfield County State of Ohio on the 17th day of January 1815.That he does not now nor has he ever applied for or received any pension from the United States for said service or any other service.That he has not
been employed or paid in the the" (sic) "Military or Naval Service of the United States since the 5th day of August 1813.That he has not in any way been engaged in or aided abetted or sympathized with the late rebellion in the United States.That he is unable to give the numbers or dates of the Land Warrants received by him for said service under act of 1850 nor can he forward here with his said Original discharge from said service for the reson the same is yet remaining at the General Land office - that he has not adhered to the cause of the enemys of the govt or exercised the functions of any office under any authority or pretended authority in Hostility to the United States."
Since leaving the service he has resided "Fairfield Perry and Sandusky CoState of Ohio & for 5 years last past in Elkhart Co State of Indiana." ....and his occupation has been "all his life a farmer" when enrolled he was a "farmer," and for the purpose of prosecuting his claim he hereby appoints "John W Irwin" of "Goshen" State of "Indiana" his attorney in fact.....He requests that his pension be paid at "Fort Wayne" State of "Indiana."His post office address is "Goshen" County of "Elkhart" and State of "Indiana."

Also personally appeared "Adam W Hartman" a resident of "Harrison Tp Elkhart" County, State of "Indiana" and "Simon P Eversole," a resident of "Union Tp said" County, State of "Indiana"persons whom I certify to be respectable and entitled to credit, and who being by me duly sworn, say that they were present and saw "Jacob Eversole sign his name" to the foregoing declaration.....that they have known him "one of them for 5 years the other forty" last past; that prior to enlistment he was a "Farmer so he represents".Since leaving the service his habits have been uniformly good, and his occupation has been "a farmer that to the knowledge of these affiants the said Jacob Eversole was not engaged in or did he aid abett or even sympathise in the late Rebellion against the Authority of the United States.Post office address of these witnesses is Goshen Elkhart Co Ind -"

The letter requests "official evidence of the enrollment, muster, service, and duty of Jacob Eversole, who enlisted" at New Lancaster OH 1st wk of April 1813 in Capt David Ewing Co, Col Wells Regt OH Militia; disch 5 Aug 1813 at Lower Seneca OH.
ITEM DATED 8 AUG 1871 -----Handwritten on Salem Bank letterhead (Jno W Irwin, Pres) Goshen INdated 8 Aug 1871to Court of Pensions, Washington "I have your printed letter dated July 3d Requiring Jacob Eversole applicant No 1861 for Survivors pension to subscribe & swear to an affidavit containing these words that 'I Jacob Eversole at no time during the late rebellion against the Authority of the United States adhered to the Cause of the enemies of the Government giving them aid or comfort, or exercised the functions of any office under any Authority or pretended Authority in hostility to the United State & that I will support the Constitution of the United States.'These are the words of the Act of 14 Feby 1871 and these very wordsYou will find encorporated in the declaration of the applicanta careful reading of the application on file will verify my statementWhy require a very feebe infirm Man to be brought 10 miles to make a statemnt the second time of what he has already done before.I very respectfully ask that a re examination be made of the said Original application to see if it be not as I claimThere will there be found Subsequently filed aff's of 2 wits proving the Same matter Altho it is not so proven by the 2 wits at the time of making the application.

ITEM DATED 8 SEPT 1871--This appears to be the Pension CertificateIt is on the form from Department of the Interior for War of 1812--Survivor's Pension.Certificate No. 4165.Only the items in quotation are handwritten on form:
I certify that, in conformity with the Law of the United States, approved February 14, 1871, "Jacob Eversole," late a ------- of Captain "David Ewings" Company "Ohio Militia" is inscribed on the Pension List Roll of the "Fort Wayne Indiana" Agency, at the rate of eight dollars per month, to commence on the fourteenth day of February, one thousand eight hundred and seventy-one.....Given at the Department of the Interior, this "Eighth" day of "September," one thousand eight hundred and seventy-"one."

I, Benjamin Benner, Administrator of Estate Jacob Eversole decd, of Capt David Ewing's Co Ohio Mil do solemnly swear that the said Jacob Eversole was the identical pensioner to who certificate No 4165 was issued ....payable at Fort Wayne, at $8.00/month;that he died at Elkhart County Ind, on the 18th day of November 1871.............Benjamin Benner, AdminGoshen, Elkhart Co IN

"Certificate of Court as to Death
State of Indiana, County of Elkhart, ss:
I, LaPorte Heefner, Clerk of the Elkhart Circuit Court in and for the County aforesaid, do hereby certify that satisfactory evidence has been exhibited to said Court that Jacob Eversole was a pensioner of the United States, residing at Harrison Tp, Elkhart (Co), Indiana and died on the 18th day of November, eighteen hundred and seventy one and that he left no wife surviving him she having died after the application for pension & before the said Jacob Eversole diedThat decedent left sons and daughters and that the said Benjamin Benner has been duly appointed Administrator of Estate of said Jacob Eversole.
Given under my hand and seal of said Court this 20 day of February, 1872.
