If the TOC or index actually gets created, I can view it, however it rarely gets that far. Instead, it fails with the "Not enough memory to create your book" error (or something similar). I can then see just the title and header, with nothing below.

Now that I think about it, the pdf problem is probably simply that the preview function can't complete properly. If the preview can't generate the book, then the export to pdf can't work.

Another detail: I have a very fast computer with gobs of memory, and yet the preview generation attempts take a very long time (several minutes, not seconds).
