Richard:
One of the participants in our WEST DNA project has exact 12/12 matches to 136 people with surnames other than WEST.It is obvious that he is not related to all of those people within a genealogical time (since the adoption of surnames). His is a case of random matches because he has a very common DNA profile (haplotype). Since your 11/12 match became a 16/25 match, it is probably in this same category. I would conclude that your two DNA test participants do not share a common male ancestor within a 500 year period. If the paper genealogy shows a relationship, there are many possible explanations for the discrepancy. We have other participants who have zero matches on the 12 marker test, their DNA profile is more rare.
