I just happened to be typesetting the Archdiocese directory and ran across the word "ephpheta;" decided to Google it for a definition, and found this forum post.
So I'm staring right at a page on my computer that says the records for Ephpheta School are at:

Archdiocese of Chicago Joseph Cardinal Bernardin Archives & Records Center
711 W. Monroe St.
Chicago, IL 60661
(312) 831-0711 Fax: (312) 831-0610

Most, though not all, sacramental or school records for the Chicago Archdiocese (Cook & Lake Counties) are there, or they can tell you which parish or mission has inherited them.
