DECLARTION FOR PENSION
The pension certificate should not be forwarded with the application.
State of Texas County of Montague on this 1st day of June A.D one thousand nine hundred and twelve(1,912)peronally apeared before me a NOTARY PUBLIC within and for the county and state a - said ,WILLIAM P DANE who being duty sworn according to the law decalres that he is 66 yrs of age and a pesident of (this is blank) County of Montague State of Texas and that he is the identical person who was enrolled at LEBDUON,KENTUCKY under the name of WILLIAM P DANE the 1st day of Sept 1863 as a PRIVATE in LOO K 13TH PEGINUAL,KENTUCKY VOLUTEER CAVARLY
the service of the u s in the CIVIL war and was honorarly dischared CAMP NELSON,KY on the 10th day of JAN 1865 not he also served 7 MONTHS AND 10 DAYS IN 14TH REGINANS KY,MOUNTED INFANTY AFTER MY DICHARGE ABOVE STATED BY RERINELILUANE
that he was not employed in the military or ? service of the us otherwise than as stated above.that his personal description at enlisted was followed :height 5 feet 9 inches
complexion dark color of eyes blue color of hair dark that his occupation was Farmer that he was born FEB 1ST 1846 at IN WAYNE COUNTY,KENTUCKY 12 MILES EAST OF MONTA?????
that his several places of residence since leaving the service have been as follows LEFT WAYNE COUNTY,KY IN MARCH 1869 AND WENT TO CRAWFORD CO,MISSOURI FROM THERE TO GRAYSON CO,TEXAS IN 1876 TO JOHNSON CO,TEXAS IN 1877 AND TO MONTAGUE CO,TEXAS SEPT 1883
that he is a penoner under certicate no 922 471 that he has (this is blank) applied for pension under orginal
that he makes his this declartion for the purpose of beins placed begins on the pension ? of the us uner the provisions of ? act of May 11,1912
that his post office address is BOWIE.RT OR L D4 BOX 36 COUNTY OF MONTAGUE State of TEXAS
