While at the library copying obits for someone, I managed to get a copy of this Card of Thanks. I'm posting it in the hopes it is helpful to someone.

We, the undersigned wish to express our sincere thanks and appreciation to our many kind relatives, neighbors and friends who by their acts of kindness, floral and spiritual offerings helped to lighten the burden of sorrow in the sickness and death of our dear mother, Mrs. Catherine Daley. Their kindness we will ever hold in grateful and loving remembrance.
