I searched a long time too, and found that xerox copies are available through

Service was slow, but the book came shortly after I e-mailed him that it was overdue.

I have found it to be very helpful and enlightening, well written and well researched. He backs up his information with quote from the Dabneys that gave him the information, or the documents mentioned. He doesn't hesitate to admit room for error, or attempt to confuse supposition with fact.
