I found this at the Irish Family History Foundation's database. It is at rootsireland.ie. The website has extensive FREE features. I used only these features to find the info in my post. If you want to view the details, there is a 5 Euro fee (about US$ 7.00) per record. This is highly cost effective if you only have one, or a few, rcds to view. Once you pay the fee, you can also print a copy of the certificate. Use high quality paper so you can keep it for posterity.
