Sorry for delay in response . . . just returned from vacation trip in Europe. My copy of the William Coolbaugh book has 43 pages, with some of them blank on the reverse side. According to my copy, here is status of your missing pages:
Page 1: Inside cover (same as outside)
Page 2: Blank
Pages 14, 16, 18, 20 are all blank
Pages 39 to 42 are out of sequence and a bit confusing to try and sort out. If you post your address, I will copy these and send them to you via snail mail. Page 43 is blank inside of back cover.
