You do not say if you know how to read and werite Spanish but trusting that someone can assist you in this regard go to www.registrocivil.cl/f_defuncion.html...but to identify his death certificate they appear to want his identity card number which leads me to advise you that there is a help desk inthe Civil registry office at 600-370-2000 in Puerto Montt where someone who speaks spanish can help you in obtaning the death certificate.
