My husband is just now ready to talk, but not about anything "upsetting" just general things about where he was, what the country looked like, people he knew- that's it. I have a brother who served 3 tours and has a lot of problems. The VA has a pamphlet about the health concerns, mostly re agent orange, it is very helpful. One I don't think is mentioned in the pamphlet is high blood pressure that is very hard to control. My husband had a stroke years ago and shortly after I heard about a study linking agent orange exposure to high blood pressure that is hard to control. They now have his blood pressure under control with diet & meds. You should drop by a VA health center & pick up their handouts- that would be one way to know without "pushing" your brother. Some of the info may be online, I have never looked.
