It is alleged that Frank J Rok enlisted 10 May 1864, as Private Co B, 138 Regt Ohio Vols; also as a Sergeant Co H, 181 Regt OH Vols from 28 Sept 1864 to 25 July 1865; was discharged at Camp Dennison, Ohio, 25 Sept 1864; while on duty at Arlington, Va., on or about 1 June 1864, he was disabled by dysentary, neurolgia of stomach, rheumatism, and injury of left eye and resulting ?eupipoeas? - was treated by regimental surgeon.

RE: No 774863 From War Dept, Record & Pension Division (to Commissioner of Pensions)
F J Rork, Co B, Reg't 138 Ohio Vols, was enrolled 2 May 1864 and M. O. with Co 1 Sept 1864 aka Frank J Rork. Frank Rork Co H, 181 Ohio Vols was enrolled 26 Sept 1864 and M O with Co 14 July 1865. From 2 May 1864 to 1 Sept 1864 he held the rank of Sergt. During that period the rolls show him present: The name of Frank J Rork, has not been found in rolls Co H 181 Ohio Vol. Other records furnish nothing additional bearing upon this case. Medical record - no record found. 25 Oct 1890.

On this 30 May 1912 personally appeared before a Notary Public, Frank J Rork; declares he is 75 years of age, resident of Claud, Curry Co NM and that he is the identical person who was enrolled at Cincinnati Ohio under the name of Frank J Rork on 28 Sept 1864 as a Sergt in Co H, 181 Ohio Volunteer Infantry; Civil War; honorably discharged at Salisbury NC on 14 July 1865. He also served as a Sergt of Co B, 138 Ohio Volunteer Infantry to serve 100 days; discharged at Camp Denison on 1 Sept 1864; served from 2 May 1864 to 1 Sept 1864.
He was not in military otherwise than stated above. Personal description: Height: 5 feet 6 inches; complexion: fair; eyes: blue; hair: dark; occupation was: Blacksmith; that he was born Hamilton County, Ohio 1837 April 6th. His residences since leaving service: Ohio, Kentucky, Indiana, New Mexico. He is a pensioner under certificate No. 587889.
