The cost for a copy of a death certificate varies, depending on if it is with or without cause. They are applied for thru the Bureau of Vital Statistics, not the county. Due to some new restrictions put in place after 9/11, you must be a relative of the person whose certificate you are seeking. I believe the cost is 9.00 for a with cause certificate, plus any research fees.

Please provide a signed letter with the following information. Applications are accepted by mail, fax, phone or in person at the State Vital Statistics office in Jacksonville.

Step 1 Determine your eligibility to receive a death certificate

Anyone may order a Florida death certificate
Limited Florida Deaths were recorded between the years of 1877 and 1917
Cause of death becomes public information 50 years following the death. Prior to that time, cause of death is considered confidential by Florida law

Step 2 Provide information about the person named on the certificate

Full name of the deceased person
Sex
Date of death (month, day, year or series of years to be searched)
City or County of death


Step 3 Provide information about the person ordering the certificate

Full name
Relationship to decedent (if requesting cause of death)
Mailing address
Phone number where you can be reached during business hours
Letters or applications must include a copy of a PICTURE ID is required for death certificates from with cause of death information only(DO NOT SEND ORIGINAL DOCUMENT) of the applicant and the signature and relationship stated. If legal guardian, please include notarized guardianship documents.
