Hello. Please have this professionally performed by someone who really knows what they're doing.
For starters, check out the Association for Gravestone Studies (http://www.gravestonestudies.org/) for general advice on working with stones (this is just so you'll know and be informed about methods, but you really should get someone who understands various gravestones).
I don't know how old your stone is, but if it's more than 70 years easily, DON'T RELY ON REGULAR EVERY-DAY MONUMENT MAKERS.They often know nothing about old stones and how to protect them, much less how to re-set them properly. These people work in thick granite and marble exclusively, and if you have anything fine and/or of different material, they could mess it up as much as you or I.
If you're in the mid-Atlantic area, I know 1 (YOUNG) gentleman whose business is solely restoring graveyards and their stones. He may even be useful for information if you're NOT in the area.He has been a member of our Coalition to Protect MD Burial Sites (www.cpmbs.org) for years now, and has given us many preservation/restoration talks/demos.
Please let me know if you need information and I hope everything works out!
