Death Certificate help

I am very confused by the death certificate I have found
on line in the Drouin section of Ancestry.

I has two names where there is usually one and the only
conclusion I can make is that they died together possibly
in an accident.

The death is in 1918 and the funeral was at the Basilique
Notre Dame, Montreal, and I believe the date is
6 April 1918 and the names are Epse Laforest and
Marie Fongeallaz. It is impossible to read the entry
of their death.

Any help anyone can offer would be greatly appreciated.
