I'm afraid I can't find my sources on this, but I can recall two versions. One that one of the BLOUNT men (perhaps Sir John BLOUNT of Sodington, b. ca 1300) married a daughter of a Lord Mountjoy. Lord Mountjoy proceeded to die without a male heir, so the *title* fell to his son-in-law. The other one is that the title Baron Mountjoy was conferred by Edward IV on a grandson of Sir Walter BLOUNT (b. ca. 1350, the one who married Sancha de AYALA) -- I forget why. At what point someone started using MOUNTJOY as a surname, I have no idea. But as far as I know, the BLOUNT's in this line continued to use BLOUNT (to this day). I know this doesn't answer your question, but maybe it will give you a clue what direction to take.
