missing birthplace in printed book form

When printing Register Book form, the birthplace is missing in some (not all) of
children listed. example:
Terry Smith b. 08 Dec 1958
Donald C. Smith b. 19 Mar 1960, Monterey, California
Larinda Lee b. 06 Feb 1962
