Zuster (sister) Maria-Theophane is indeed an adopted name.
"in de wereld" literal means "in the world" - it usually is put before the maiden name of a nun.
"Kanunnikessen" is plural for "kanunnikes". "Kanunnikes" is female for "kanunnik". "Kanunnik" means "Canon" - it is a title given to some religious people. "Kanunnikessen van den H. Augustinus" is a religious order in Flanders.
Mother (moeder) means that this nun was a superior in the convent.
Eerw. is an abbreviation for "Eerwaarde", is means "Reverend".
