Most of the time the baptismal certificate will not contain a place of birth. It is the church baptismal RECORD that will have mother's maiden and as well as parents place of birth.
The church should be either Russian Orthodox or Byzantine Catholic [Greek Catholic is a term used in old country] but not the way you have it.
MICHAYLOVA sounds like MICHAJLOVA [Michael's]
SAVCHUK is an americanized version of SAWCZUK.
