The document is in Russian because in 1912 central part of Poland was under Russian occupation. "Borkova" is probably Borkowo ( Borkowo Koscielne) near Sierpc , abt 150 km N from Warsaw. It was in Plock district (Gubernia). Kishelyova can be the word "Koscielne" which is the second part of the full name of the village.
