Hi Ada:
My thanks to everyone who responded to my first posting. I should have mentioned that I have reviewed "Jerry's" Social Security Death Index record and it reveals: "JEREMIAH ALLEN (82) Apr 1874 - Nov 1956 (not specified) (none specified) 359-07-8173, Illinois". That document incorrectly provides his DOB as "1874" and does not "specify" where he died.
I have also obtained a copy of Jerry's "Application for Social Security Number" which, of course, does not reveal information about his death but does contain other information about his life.
From other records, I know that Jerry was born 22 Apr 1878 at Marshall County, IN, and that he died about 1956 while residing in an Old Age Home located somewhere in Arkansas.
My dilema is that I cannot determine where Jerry died and the exact date of his death. Officials at Arkansas Department of Health, Division of Vital Records, have refused to research their files and provide a copy of his Death Certificate unless I can provide all essential information.
Is there anyone out in cyberland who can help?
