The Birmingham Public Library has a database they made of
investigated mining accidents which lists him. The reports
are likely at the state archives. I'd write the BPL here
http://www.bplonline.org/ask/asklibrariangenealogyform.htm
And see what they could tell me about their source.

Last Name: Hazelwood First Name: William
Race: white
Mine: Acmar No.3
Cause: explosion of gas
Date of Accident: 8/24/1925
