I visited the Old Mt Lebanon Cemetery today. It is on private property and it is necessary to have a four wheel drive to get there. You also need permission from the owner of the property. I made pictures of most of the graves and will send you a copy of the picture of the grave you mentioned if it is one of the ones I made. The church is gone but the owners of the property or someone keeps the graves cleaned off. The owner's son mentioned that there is a decoration day some time in May. He did not say exactly when. I did not have any relatives in the cemetery but I made pictures just in case someone else wanted them.
The cemetery is just over the county Fayette/Walker County line in Fayette County.
