I was born 11/23/1939 [7:55 pm] under the name of Mary Elizabeth Phillips, at Maple Knoll Hospital, Village of Glendale, County of Hamilton, Township of Springfield, OHIO. Attending physician was Dr J V McGowan of Carthage Ohio. My adopted parents were Ralph and Mildred Kalp of Progress Ave., Hamilton Ohio. My name was changed to Kay Lenore Kalp. Would like to have any information regarding my past. My adoption records are permanently sealed.
